Preferred vs. Standard Pharmacies in Medicare Drug Plans

Preferred and standard pharmacies may both be in a Medicare drug plan’s network, yet the preferred location can offer lower cost sharing for some prescriptions. Pharmacy status can materially change estimated annual drug spending.

How pharmacy status changes your price

A plan contracts with network pharmacies and may designate some as preferred cost-sharing pharmacies. The same medication, dosage, and quantity can cost less at a preferred location than at a standard network pharmacy. Status varies by plan and year, so a pharmacy that is preferred for one plan may be standard—or out of network—for another.

Practical checklist

  • Price the same complete drug list at multiple nearby pharmacies using the exact plan, dosage, quantity, and refill frequency.

  • Compare 30-day and 90-day supplies, retail and mail-order options, and whether the deductible applies.

  • Confirm preferred status for the correct plan year; do not rely only on the pharmacy’s statement that it “takes” the carrier.

  • Review specialty-pharmacy, limited-distribution, delivery, prior-authorization, step-therapy, and quantity-limit requirements.

The same chain can have different plan status

Do not assume the closest or most familiar pharmacy offers the lowest cost. Review the plan’s current directory and formulary, then confirm participation with the plan and pharmacy. Recheck during Annual Enrollment because contracts and preferred status can change.

Compare retail, mail-order, and refill costs

Price every prescription at preferred and standard pharmacies, including deductible stages, day-supply limits, and mail order.
