Medicare drug formularies and pharmacy networks

A Medicare drug plan’s formulary and pharmacy network work together to determine whether a prescription is covered, which rules apply, and what you may pay. Checking only the drug name—or only the monthly premium—can miss important cost differences.

What is a formulary?

A formulary is a plan’s list of covered prescription drugs. It includes brand-name and generic medications, organizes them into cost-sharing tiers, and identifies requirements such as prior authorization, step therapy, or quantity limits. Formularies differ between plans and can change for a new plan year, subject to Medicare rules.

Drug tier

The tier assigned to a medication can affect its copay or coinsurance, but the same tier number can have different costs in different plans.

Coverage rules

Prior authorization requires approval; step therapy generally requires trying another covered drug first; quantity limits restrict how much the plan covers during a period.

Exceptions

You or your prescriber may request a formulary or tiering exception when medically appropriate. Approval is not guaranteed, and timing and appeal rights depend on the request.

Why the pharmacy matters

In-network pharmacies contract with a plan. Preferred in-network pharmacies may offer lower cost sharing than standard network pharmacies, while mail-order or specialty-pharmacy rules may apply to certain drugs. Out-of-network prescriptions are generally not covered except in limited circumstances described by the plan.

  • Enter the exact drug name, dosage, quantity, and frequency.

  • Compare your local pharmacies and any preferred-network pricing.

  • Check whether mail order is available and appropriate.

  • Review estimated annual costs, not only the plan premium.

  • Repeat the comparison each year because formularies and pharmacy arrangements can change.
