Doctors and specialists

Prescriptions

Write down each medication’s exact name, dosage, frequency, and whether you prefer a retail or mail-order pharmacy.

Current coverage

Bring your Medicare card, current plan information, employer or retiree benefits, Medicaid details, and notices from your insurer.

Preferred pharmacies

Plan pricing can differ between standard, preferred, retail, and mail-order pharmacies. Include the locations you actually use.

Travel and residence
