How Medicare Prescription Drug Tiers Affect Your Costs

Medicare Part D formularies place covered medications on tiers that often have different copays or coinsurance. The tier matters, but the deductible, pharmacy type, drug quantity, coverage phase, and utilization rules can affect the final cost just as much.

How drug tiers change pharmacy costs

Tier structures differ by plan. A generic drug may be placed on a lower tier, while preferred brands, non-preferred drugs, and specialty medications may use higher copays or percentage-based coinsurance. Never compare tier numbers alone: price the exact drug, strength, form, quantity, and frequency under each plan for the correct year.

Practical checklist

  • Search the exact drug name, strength, dosage form, quantity, and refill frequency; similarly named products may be treated differently.

  • Check whether the tier uses a fixed copay or coinsurance and whether the drug deductible applies before that amount.

  • Compare preferred, standard, mail-order, and specialty pharmacies because pharmacy status may change the same drug’s cost.

  • Look for prior authorization, step therapy, quantity limits, and whether your prescriber may request an exception.

Tier placement can change by plan

A low premium does not guarantee low prescription costs. Review the official formulary and Evidence of Coverage, confirm current pharmacy participation, and calculate estimated annual spending across all medications. Recheck every year because tiers, restrictions, and pharmacy contracts can change.

Check every medication and restriction

Compare each medication’s tier, restriction, pharmacy price, and available lower-cost alternatives with your prescriber.

Compare costs at your pharmacies

Estimate costs using each medication’s tier, quantity, pharmacy, refill schedule, and any utilization requirements.

Use the current formulary
