What Does Accepting Medicare Assignment Mean?
In Original Medicare, a provider who accepts assignment agrees to accept the Medicare-approved amount as full payment for a covered service. You remain responsible for applicable deductibles and coinsurance, but the provider cannot add a higher charge for that service merely because its usual fee is higher.

Participating and non-participating providers
Participating providers accept assignment for all Medicare-covered Part A and Part B services. Non-participating providers may accept assignment case by case and, where permitted, may charge up to the limiting charge. Providers who opt out of Medicare generally use private contracts, and Medicare usually does not pay for their services except in limited emergencies.
The limiting charge
Medicare explains that, in many cases, a non-participating provider’s charge cannot exceed 15% above the Medicare-approved amount. The rule does not apply identically to every service or situation.
Providers who opt out
An opted-out provider generally uses a private contract rather than billing Medicare for covered services. Ask about status and expected payment before non-emergency care.

Keep copies of notices, confirmations, and cost estimates.
Ask the provider for a written estimate and private-contract terms when Medicare assignment or opt-out status is unclear.
