Identity and eligibility information

For an official Medicare application, be ready with your legal name, date and place of birth, Social Security information, current address, citizenship or lawful-presence information when applicable, and details about railroad employment. Share Social Security and Medicare numbers only through official secure channels or with trusted authorized parties.

Current coverage

Record the exact employer, union, retiree, COBRA, Marketplace, VA, TRICARE, Medicaid, or other coverage held by you or your spouse. Include the policy name, member number, employer size, active-employment status, whether prescription coverage is creditable, and the date employment or coverage may end.

Care and prescription list

List each doctor, specialist, hospital, and facility with its location. Record every medication with dosage and frequency, preferred pharmacies, planned procedures, durable medical equipment, recurring services, travel patterns, and seasonal addresses. These details support provider-network, formulary, pharmacy, and total-cost comparisons.

After the Medicare card arrives
