Medicare Appeals and Coverage Decisions: A Practical Guide

If Original Medicare or a Medicare plan denies coverage or payment, read the written notice immediately. It identifies the decision, reason, filing deadline, and first appeal step. Appeal paths differ for Original Medicare, Medicare Advantage, and Part D, so follow the instructions for the coverage involved.

Why appeal deadlines require quick action

Keep the notice, medical records, prescriptions, receipts, and notes from calls. Ask the treating clinician to explain why the service or drug is medically necessary and address the plan’s stated reason for denial. If waiting for the standard timeframe could seriously harm health, ask whether an expedited decision or appeal is available.

Practical checklist

  • Read the denial notice immediately.

  • Preserve the filing deadline.

  • Submit relevant clinical support.

  • Request expedited review when delay could harm health.

A phone call does not replace a formal appeal

Follow the written notice. Informal calls do not preserve an appeal deadline or replace the required submission.

When coverage is denied

Build the supporting record

Match the appeal evidence to the denial reason and ask the treating clinician for specific medical support.

Ask about an expedited review when needed

Track filing deadlines, required documentation, temporary coverage rules, and your expected cost while an appeal is pending.

Follow the notice and current Medicare rules

Use Medicare’s current appeals guidance and do not miss the deadline printed on the notice. A State Health Insurance Assistance Program can provide free counseling. For plan-rule preparation, see our guide to prior authorization and referrals. This is educational information, not legal advice.
