Maximum Out-of-Pocket Limits in Medicare Advantage Explained

Medicare Advantage plans set an annual maximum out-of-pocket limit for covered Part A and Part B services. After you reach the applicable limit, the plan generally pays 100% of covered Part A and Part B services for the rest of that calendar year. The limit is not the same as the amount you should expect to spend.

Why the maximum matters even if you never reach it

Compare both the in-network limit and, for plans with out-of-network coverage, any combined limit. Confirm which deductibles, copays, and coinsurance count. Monthly premiums, Part D prescription costs, noncovered services, and costs that do not follow plan rules generally do not count toward the medical maximum.

Practical checklist

  • Compare the in-network maximum.

  • For PPOs, examine out-of-network limits.

  • Identify which cost sharing counts.

  • Remember Part D drug costs follow separate rules.

Not every expense counts toward the limit

The advertised maximum does not include every health expense. Confirm which medical cost sharing counts and which costs follow separate rules.

Before comparing plan limits

Separate medical and drug spending

Estimate how your expected services accumulate toward the medical limit and which expenses remain outside it.

Model likely and worst-case costs

Add likely copays and coinsurance until the in-network maximum is reached, while keeping premiums and excluded services separate.

Use the current Evidence of Coverage

The federal ceiling can change, and individual plans may set a lower limit. Use the current Summary of Benefits and Evidence of Coverage, then model likely spending—not only the worst case. See Medicare’s coverage comparison and our Medicare costs guide.
