How Medicare Advantage Provider Networks Work

A Medicare Advantage network determines which doctors, hospitals, pharmacies, and other providers participate in a plan—and what you may pay when you receive care. Understanding the difference between HMO and PPO network rules can help you avoid unexpected costs and disruptions.

How network rules affect access to care

A carrier may offer several Medicare Advantage plans in the same county, and each plan can use a different provider network. A doctor who accepts one plan from a carrier may not participate in another. Network status can also change during the year, so always confirm the exact plan name and contract—not only the insurance company—before enrolling or scheduling non-emergency care.

Practical checklist

  • Verify every important doctor, specialist, hospital, laboratory, and facility against the exact plan’s current directory, then confirm directly with the provider’s billing office.

  • Check whether the plan requires a primary care physician, referrals for specialists, or prior authorization before certain services, procedures, or equipment.

  • Compare in-network and out-of-network cost sharing. HMOs generally provide routine coverage only inside the network, while PPOs may permit out-of-network care at a higher cost.

  • Confirm the specific practice location you use. Participation may differ between offices, facilities, individual clinicians, and medical groups.

A directory listing is not a guarantee

Do not rely only on a provider’s website, a marketing brochure, or a statement that the office “takes Medicare.” Accepting Original Medicare is different from participating in a particular Medicare Advantage network. Use the plan’s directory, call the plan, and confirm with the provider. Save the date, representative’s name, and reference number for your records.

Before choosing a network-based plan

Confirm each provider and facility

List the doctors, specialists, hospitals, pharmacies, durable medical equipment suppliers, and travel patterns that matter to you. Ask how referrals, telehealth, urgent care, and care away from home work under the exact plan.

Compare in-network and out-of-network costs

Compare premiums, deductibles, copays, coinsurance, out-of-network charges, and the annual maximum out-of-pocket limit. A broad network can still be a poor fit if the providers you use are not included or key services carry higher cost sharing.
